Primary Care at Scale
Delivering More Together
Stort Valley Healthcare supports the delivery of primary care at scale by bringing together practices, workforce, expertise and resources to meet the needs of our local communities.
As a GP Federation, we help practices work collectively to improve services for patients while supporting the wider health and care system to deliver better outcomes for local people.
Why We Work at Scale
Stort Valley Healthcare was established to help practices work together for the benefit of patients and communities.
Bringing together workforce, expertise and resources enables the development of services, initiatives and models of care that would be difficult to deliver at individual practice level alone. By working collaboratively, we can improve access, strengthen resilience and support better outcomes for local people.
Working at scale also enables us to develop innovative approaches to healthcare, support service sustainability and work more effectively with partners across the wider health and care system.
Through collaboration, clinical leadership and shared expertise, we help deliver services that respond to local need while supporting the delivery of more integrated, coordinated and sustainable healthcare for our communities.
