Stort Valley Healthcare is a GP Federation that designs, delivers and enables integrated healthcare services across our communities.

Working alongside practices, providers and partners, we deliver healthcare services directly, lead collaborative programmes and support the development of innovative models of care that improve outcomes for local people.

We work on behalf of local practices and in partnership with NHS organisations, community providers, local authorities and voluntary sector partners to improve population health, reduce inequalities and ensure people receive the right care, at the right time, in the right place.

Our focus is on delivering high-quality services, improving access to care, supporting people to stay well and creating a more joined-up experience of care.

Providing leadership, management and operational support for the Stort Valley & Villages Primary Care Network, enabling the delivery of integrated services and multidisciplinary care across the locality.

Integrated Urgent Treatment Centre

Joint lead provider of the Integrated Urgent Treatment Centre in Harlow, working with partners to deliver timely, high-quality urgent care.

Enhanced Health in Care Homes

Managing the Enhanced Health in Care Homes (EHCH) contract across East Hertfordshire and working with partners to improve outcomes, enhance quality of care and reduce avoidable hospital admissions for care home residents.

Healthcare Transformation & Innovation

Bringing organisations and professionals together to improve outcomes and transform services.
